Criminal History and Consent for Background Check

This section is crucial as our program involves interactions with individuals on probation, parole, and visits to jails and prisons. We prioritize the safety of both potential mentors and the members of our program. Understanding your criminal history, if any, and any pending criminal charges is essential for making informed decisions that ensure the safety and well-being of all participants in our program. Your cooperation with a background check demonstrates your commitment to maintaining a secure and supportive environment within our mentorship program.
Have you ever been convicted of a crime?
Are there currently any pending criminal charges against you that could potentially restrict or limit your ability to visit jails or correctional facilities as part of the mentorship program?
We require all mentors to undergo a background check. Are you willing to consent to a background check?
