What We Are Doing

We are soliciting your Christian testimony or personal story to be used in an inspiring publication about how God works in the lives of Christians. Our hope and goal is to inspire faith in Jesus Christ. The accounts of GOD working in our lives provides us, and others, with the hope, confirmation, and encouragement needed in today’s challenging world.

What Is a Testimony or Personal Story?

A testimony or personal story is an account of our connection with God. It can be about your journey to becoming a Christian, or a way in which God has shown up in your life. Perhaps it is a lesson God has taught you or a reminder that He gave you. Sometimes, it’s an answered prayer, a victory over sin, or a specific event in a Christian’s life in which God did something deemed particularly special to share.

How You Can Be Included

If you agree to the terms below, this release allows us to use your personal story in the publication. To be included, follow these simple instructions:

• Say a prayer that this publication will be of encouragement to someone in need
• Complete and Submit the below agreement
• Write your Christian testimony. Your testimony will be quoted, where appropriate, and many be edited (for writing style but not content). You may include an original photograph or other likeness if needed to illustrate your personal story.
• Indicate the name you wish to be associated with your personal story (John Doe; John D.; J. Doe, etc.)

Personal Story Release Agreement

This agreement ("Agreement") is entered into on the date submitted by both parties described herein. The Lord's Heritage Church ("Company") and the submitter of this form ("Storyteller") do hereby agree to all the terms described and given below.

Grant

For consideration which I acknowledge, I consent to the use of my statements and grant Company and Company's assigns, licensees, and successors the right to copy, reproduce, edit, and use all or a portion of the statements (the "Personal Story") for incorporation in Company's publication ("the Work").

I grant the use of all or a portion of the Personal Story in the Work and any of its derivatives in all forms and media, electronic, print, and other, including advertising and related promotion worldwide and in perpetuity. I grant the right to use my name or pseudonym along with my Likeness (if provided) in connection with all uses of the Personal Story and waive the right to review or approve use of my Personal Story as incorporated in the Work.

Release

The Storyteller releases the Company and Company's assigns, licensees, and successors form any claim that may arise regarding the use of the Personal Story including any claims of defamation, invasion of privacy, or infringement of moral rights, rights of publicity or copyright. I acknowledge that I have no ownership rights in the Work.

The Company is not obligated to utilize the rights granted in this Agreement.

I am NOT ENTITLED to any royalty or commission on the sales of the Work.
